Outcomes reporting — in progress.

We're finalizing our current outcomes report from HOC's tracking system. Rather than publish figures we can't yet stand behind, we're holding them until every number is verified. Referring partners and donors can request our latest available data directly — we'll send it.

What we measure

The report covers all of this.

When it publishes, our outcomes report will cover the metrics below. We're listing what we track now, and holding the figures until they're confirmed.

  • Successful discharges (trailing 24 months)
  • Discharge destination — reunified, kinship, step-down, independent living
  • Family satisfaction (post-discharge survey)
  • Treatment goals met at discharge
  • Average length of stay vs. industry benchmark
  • Reduction in incident frequency, intake to discharge
  • Academic progress — on-track rate, credits earned, IEP goal attainment
  • Post-secondary enrollment among graduates
  • Alumni follow-up at 1, 3, and 5 years post-discharge

How we measure

What we count. Discharge outcomes assessed at the time of discharge, classified by the treatment team in coordination with the case team. Family-satisfaction surveys distributed within 30 days of discharge with one follow-up at 90 days. Alumni follow-up surveys distributed annually to consenting graduates at 1, 3, and 5 years post-discharge.

What we report. All metrics reflect a trailing 24-month window unless otherwise noted. Survey response rates are disclosed alongside every figure in the full report — we don't publish a percentage without the sample it came from.

What we exclude. Residents discharged within 30 days of admission are excluded from outcome metrics. Residents whose case-team disclosure preferences exclude post-discharge follow-up are omitted from alumni metrics.

Full report. Available to referring partners and donors on request — including year-over-year trend lines, demographic breakdowns where statistically meaningful, and the questions we're actively trying to answer.
